We provide a wide range of primary, community and intermediate healthcare services, including community nursing, child health services, occupational therapy, palliative care and a wheelchair service Our main providers of services are Northumbria Healthcare NHS Foundation Trust, Northumberland, Tyne and Wear NHS Trust and Newcastle upon Tyne Hospitals NHS Foundation Trust We commission social care services for adults and agreements are in place with a range of service providers, including our own services, from other NHS trusts and other statutory, voluntary and independent sector organisations We were established in 2002 following the dissolution of Northumberland Health Authority and were the first primary health, community health and social care based trust in England and Wales The social care we provide includes care management services for older people, older people with mental health problems and for adults with physical or learning disabilities
