Care At Home
When we receive a request to provide services, we will record the details already provided and arrange to visit you at home. This first visit is to meet with you and members of your family if you wish to find out what help you may need. If you are happy for us to support you, we will then assess your needs which involves asking you questions about the kind of help you may need. These details will then be written into your support/care plan and a copy given to you along with your needs assessment and risk management plan. We then match your individual needs and support requirement to the training and experience of our support workers assigned to help support you.
